Hotel
September is a popular month for visiting Amsterdam. So hotel rooms may not be available when booking after 1 September 2008. Hotel reservations made after 1 September 2008 are at conference prices but on a room available basis. You can use the hotel reservation form and fax this directly to the NH Barbizon Palace hotel +31 20 554 44 00.

Payment
Full payment must be received with your registration form. Charge your registration to American Express, Visa or MasterCard or by bank transfer not later than the date of receipt of your registration. Please note that only registrations with complete payment will be processed.

• In person on-site: iIf you are unable to register on or before 25 September 2008, please register at the on-site registration desk at the NH Barbizon Hotel. To expedite your registration process, please bring a completed registration form with you. However, you do run the risk that we will be fully subscribed by then so we advise you to book in advance.

Confirmation
All registrants will receive confirmation by e-mail.

Badges
For those attendees registered in advance, badges and registration materials will be available at the LES Registration Desk in the NH Barbizon Palace.

Registration list
Registrations received before or on 1 September 2008 will be included in the meeting participants list.

Speaker presentations
Most speaker presentations will be available for downloading at the meeting website www.2008.les-benelux.org after the conference. All registrants will receive a password after the conference by e-mail.

Cancellation policy
All cancellations must be received in writing. All refunds are subject to Euro 100,00 administrative fee. All registration fees are non-refundable after 1 September 2008. If the registrant is unable to attend, another person may substitute upon written request. Substitutes within 30 days are subject to Euro 100,00 administrative fee.

Accompanying person policy:

The conference registration fee covers both registrant and one accompanying person. Licensing professionals may not attend as accompanying persons. Accompanying person refers to a personal relationship, not a business associate or colleague.

Insurance Conference

Participants are strongly advised to carry the proper travel and health insurance. As LES cannot assume any liability for accident, injury, illness and the like, nor for the inability of a participant to attend the conference for any reason whatsoever (including force majeur).

Registration list policy
By registering for this conference, you agree that LES may maintain your name and employer information completed in this form, as necessary, for its activities including publishing on the conference website and in a list of registrants available to the organizers and meeting attendees. Reproduction of the registrants list is not author ized and its use for general mailings or similar purpose not specifically authorized by LES Benelux is prohibited.

Special requirements
Participants with special requirements in respect of diet or other matters are requested to inform the conference secretariat well before the conference.

Applicable law and jurisdiction
The relationship between conference registrant (and any accompanying person) and LES Benelux and/or Assistance Office SWAN is governed by the substantive law of the Netherlands and subject to the exclusive jurisdiction of the competent courts
in Amsterdam, the Netherlands.
